This series documents the massive number of deaths and injuries caused from preventable medical errors. These errors cause "more deaths than traffic accidents', more specifically they cause nearly "200,000 deaths per year" Behind the numbers are the people who trusted the medical system, including the 30 individual cases spotlighted in this series. Furthermore, once the problems were revealed the medical community and the government failed to take the effective steps necessary to solve the problems.

Death on the Rails
When nine people died and 80 were injured on Washington's Metro June 22, the general manager described the event as "a freak occurrence." However, the article revealed that the Metro's automatic "fail-safe" system had failed repeatedly before the collision, but past failures barely escaped catastrophic consequences.

The investigation "uncovered a plan that began in the mid-1990s with the help of one of the nation's leading consultant firms, McKenzie, to force motorists to sue to recover costs for so-called soft tissue injuries. Led by the nation's two leading insurers, State Farm and Allstate, insurance companies developed a strategy of delay, deny and defend when it came to minor car crashes."

Danger in Tow
Los Angeles Times investigated U-Haul International. They looked at the high number of towing accidents that occur while towing U-Haul trailers. U-Haul trailers were not kept up to standard and maintenance was poor. When faced with law suits from injured customers, "U-Haul has repeatedly lost, altered or discarded truck and trailer parts" which would have been evidence.

Certified Dangerous: Used Cars' Airbags
An investigation of 1650 consumer complaints about the failure of airbags to deploy during car accidents such as head-on collisions, rollovers and broadside crashes. Some airbags are not replaced when the vehicle is being fixed after an accident, leaving the driver more succeptible to injury.

Harvest of Death
The story investigated the disproportionately high number of auto fatalities and injuries caused by Hispanic drivers, most of them seasonal migrant workers, on Virginia's East Shore. Most of the accidents were alcohol related.

Military Menace: Deadly Vehicles
Many of the deaths and injuries to American soldiers in Iraq are the result of vehicle accidents rather than bullets or bombs. Zagaroli's series examines how military vehicles are often out of date, do not have standard safety equipment, and are being driven by soldiers with little or no training. Because the Pentagon did not send enough armored vehicles to Iraq, soldiers fitted their own Humvees with make-shift armor that the vehicles were not designed to carry, which made them more accident prone.
