  • Abuse and Neglect of the Brain Injured

    These stories revealed a disturbing pattern of abuse and mistreatment of severely brain-injured people in the United States. At one of the largest rehabilitation facilities in the country, Bloomberg uncovered a decades-long history of death, abuse and neglect. Another story reported on thousands of other brain-injured patients warehoused in nursing homes with little or no treatment and in conditions that ranged from filthy to dangerous.

  • Patient Safety Crisis at Parkland

    This investigation takes a look at Parkland Memorial Hospital, which mostly treats Dallas' most vulnerable patients, the poor and the elderly. The findings are shocking and extensive, including patient neglect, unsupervised practices from doctors in training and poorly trained psychiatric technicians.

    Florida was among the first to regulate assisted living and to adopt a Bill of Rights for patients. The investigation found that regulators are failing to protect some residents and uncovered dozens of questionable deaths.

  • "Deaths at the State Hospital"

    This ongoing investigation reveals major misconduct by the Colorado Mental Health Institute in Pueblo, the largest public psychiatric hospital in the state. The investigative team exposed and detailed the deaths of four patients that resulted from the "mistakes, lack of training, incompetence and possible criminal neglect" carried out by hospital employees. The series also reveals the attempt of state human services officials to cover up the mistakes.

  • Undercover Inside Ghana's "Mad House"

    A reporter spends seven months undercover in Ghana's major psychiatric hospital. In a series of four stories, he uncovers the "neglect and abuse" of patients by the staff, as well as the purchase and distribution of narcotics within the hospital walls.

  • Mental Disorder: The Failure of Reform

    Until the News and Observer published "Mental Disorder," most North Carolinians had no idea that their state mental health system was a disaster. The five-part series examined each major failure of an 8-year reform effort. Major findings included that the sate had wasted at least $400 million on services that were ineffective or unneeded and various cases of money mismanagement. They also found that at least 82 patients in state mental health hospitals and homes for the developmentally disabled had died of homicide, suicide, accidents or medical errors. In dozens of cases, hospital officials had covered up the true circumstances of the deaths by falsifying records and telling family members the patients had died of natural causes.

  • Trouble at Delaware Psychiatric Center

    If the nurse at the Delaware Psychiatric Center (DPC) report the patient abuse or neglect of the lesser-trained attendant staff, their cars would be vandalized and they would suffer other forms of retaliation. One patient had his jaw broken in three places due to an attendant assault, several females reported they’d been raped in DPC, and questions of the criminal backgrounds of doctors were raised.

  • No Haven For the Elderly

    This series "exposed significant patient-care deficiencies at one of Connecticut's largest nursing home operators, Haven Healthcare." Not only did the Courant's reporters discover neglect and a "deeply flawed regulatory system," the owner of the Haven Healthcare chain was using money to fund personal endeavors even while the chain fell into debt and could not pay for basic things.

  • A Hidden Shame: Danger and Death in Georgia's Mental Hospitals

    This series exposed problems in Georgia's state psychiatric hospitals. At least 155 patients died under suspicious circumstances related to neglect, abuse and poor medical treatment. Furthermore, patients are often discharged to places where their continued treatment is doubtful, such as homeless shelters, bus stations and street corners.
